The Jacobi Medallion honors a Mount Sinai alumnus/alumna or current colleague who, through long-term service, has made exceptional contributions to the fields of medicine or biomedical research, or has exemplified extraordinary dedication to The Mount Sinai Alumni Association. The names of previous awardees can be found at

This year is very special because we have identified that there are currently over 25,000 Mount Sinai alumni and among this highly accomplished and impressive group of physicians, scientists and educators, are many who are worthy of the Jacobi Medallion.

We have extended the deadline to encourage the submission of nominations of candidates from this larger group of eligible alumni, including biomedical scientists and health professional, full time faculty, voluntary faculty, who are outstanding in one of the following categories:

  • Achievements in Biomedical Science
  • Achievements in Education
  • Breakthroughs in Clinical Care
  • Exceptional Contributions to Mount Sinai

These nominations will be added to those that have already been submitted for our outstanding current or former colleagues.

Please submit your nominations online at no later than October 12, 2015 by 5 p.m. You may nominate up to four candidates you feel are deserving of this honor.

Completed nomination submissions will include:

 • A letter of nomination (required)

 • The nominee’s CV or resume or bio (required)

 • Up to three letters of support for the nominee from other members of the current or past Mount Sinai community (optional).

Incomplete submissions will not be considered.

The awards will be presented at the annual Jacobi Medallion ceremony, reception, and awards presentation on Wednesday, March 16, 2016.
If you have any questions regarding the nomination process or are experiencing difficulty using the online submission portal, please contact the Office of Alumni and Development at 646-605-8717 or
